Service & Warranty Request – Instruments & Equipment

Before you begin:

• One form per product — you can start a new form immediately after completing one.
• This form is for abutments, healing caps, instruments, equipment and other non-implant products. For implants, please use the implants module.
• You will be asked to upload a photo of the product label or serial number, and optionally a photo of the defect. Complete this form on the device where you can take those photos.
• Depending on the product you can ship it yourself or request a return label. For equipment and other products our Customer Support will contact you to arrange the return.
• Completing a form takes approximately 1–3 minutes.
